Your GP or another health professional can refer you directly to the Gender Identity Clinic.
We also accept self-referrals however your GP needs to be involved.
We accept referrals from all over Britain (except Scotland and Northern Ireland) for patients who are aged 17 years and over onto our waiting list.

If you are a GP or another health professional and you would like to refer a patient to the Gender Identity Clinic, please complete the referral form and either refer using eRS or email or post it to the address below:
Electronic Referral Service
To locate the Gender Identity Clinic service on eRS, enter the following in the search fields:
- Priority = Routine
- Specialty = Mental Health – Adults of all ages
- Clinic Type = Not Otherwise Specified
- Organisation or Site Name field = Tavistock and Portman
Should you wish to be self-referred to the Gender Identity Clinic you are required to be registered with an NHS GP/healthcare professional.
If you are submitting the referral as a self-referral, please note that you would still require your GPs involvement in providing us with your medical assessment.
The GP’s involvement is required throughout the whole treatment so it is advisable that you have a GP who will support you through this pathway.
Please complete the referral form and either:
Submit to your GP for them to complete the medical assessment and once completed submit to the GIC via email or post
Submit to your GP for them to complete the medical assessment and once completed ask them to return to yourself and you can submit to the GIC via email or post
