We aim to provide the highest quality care. We want to hear from you about your experiences of our service and any suggestions you might have so that we can continue to improve and provide a service that is right for you.

If you have a suggestion for us, a compliment, or an enquiry about the service, please use our Patient Advice and Liaison Service form on our main Tavistock and Portman website.

If you wish to make a complaint, please complete the complaints form on our main Tavistock and Portman website. We take all complaints very seriously. Please be assured that your care will not be adversely affected if you raise a complaint.

It is important to think about what you want to happen as a result of your complaint, whether made orally or in writing. Do you want an apology? An explanation? To see changes or improvements made? Do you want people to acknowledge their mistakes? Assurance that the same thing cannot happen again? This helps ensure we understand what actions might resolve your complaint.

We know this is a difficult time for many. However, verbal or physical abuse of any of our staff will not be tolerated, whether in-person or via telephone/email.

Abuse of our staff could result in discharge from the service or prosecution.
